This is a Ceropegia stapeliiformis. You should water it and fertilize it in the warm weather. It needs part sun part shade. Let the soil dry out before watering. You should water it more frequently in the spring and summer months. You should fertilize it about once a month. I hope this helps ya!
Propagate it
I agree, at this point propagate it, it’s way too veiny, cut the leaves you have left so that you include the nodes and place in water for couple of weeks, when they develop roots, plant them in a new pot and you’ll have a brand new plant.
