Overwatered or underwatered plant?

What's wrong with my orchid?

I was given this orchid on February 14 and it was beautiful. But I have days noticing that all the new shoots that want to come out die and I noticed gray roots that I just trimmed that I showed in a photo, but inside I notice some black and white lumps I don't know what to do :(

water propagation going well

I think the plant as a false shamrock I accidentally ripped off the leaf thats diseased. Does anyone know what type of disease it is and how I can treat my plant none of the other leaves look diseased, but this has happened to two of the leaves on the plant if anyone has any advice, Id appreciate it

Suddenly Leaning

Once it reached a certain hieght it just started leaning, I also noticed some discolouration and dead leaves. This is day 1 in new location... hoping to bring color back and correct that curve :)

slow growth, does it need bigger pot?

Not knowing anything about this plant. Its been slow growing. Is this normal for this plant or would it grow better in a bigger pot? Thoughts? I had another plant (Madagascar jewel) that grew faster when I put it in a bigger pot. Wondering if this is kinda the same. Its not like its rooting out or anything. But just seems stunted.

how it started to now

Its taken almost 2 years for this plant to flower. And finally I have a bloom and a 2nd bud. Plus 3 more blooms starting!!! It was straggly looking when I got it. And about 6 months ago I decided to cut off the lengthy parts and try and propagate it. And it worked! Repotted and added the regrows in and this baby blossomed afterwards 😍
