It’s not been growing and it’s pretty much dead, what’s wrong with it?

I think you need to move it to a larger pot!

It honestly depends, based on the leaves it could mean over watering, or root rot depending how often you water it

Change the soil in to new soil. Bit more Bigger pot and some holes under for water drainage. Give some plain rice water every 4 days 1 cup.
